基礎疾患なく突然の脳室内出血にて発症し,右側脳室内動脈瘤が認められた55歳女性の症例を報告した.動脈瘤は,前大脳動脈A1穿通枝内側群より発生し,モンロー孔直上の脳室壁に存在したが,直達手術により摘出し,良好に治療しえた.A1穿通枝を親血管とする脳室内動脈瘤は過去に報告例がなく,きわめて稀な症例と考えられた.
