Peritoneal pathophysiology after PD treatment has remained unclear. Thus, this study investigated the longitudinal changes of peritoneal permeability after PD. Five patients who underwent long-term CAPD treatment were studied (male/female: 1/4, mean age 52.4 years old, mean duration of CAPD: 99.4 months). In all cases, CAPD catheters remained in place, and peritoneal equilibration test (PET) was performed monthy thereafter. All patients performed intra-abdominal lavage once a day using a standard PD solution during the test periods (mean duration 8.8 months). The creatinine Dialysate/Plasma ratio obtained by PET during the study periods showed the following results; rapid decline in accordance with the post PD duration (two cases), sustained elevation during the study periods (one case) and slow decline after the initial elevation (two cases). The results suggest that there are, at least, two groups with respect to change in peritoneal permeability after long-term CAPD treatment; the increased and the decreased permeability groups. The clinical implications of this pathophysiology remains unclear and it needs further study.
