A 54-year-old woman was admitted on April 1993 because of hyperglycemia, fever, and leg edema. She had marked onychogryposis with paronychia, and so-called “akatsuki disease”. Onychogryposis is uneven growth of a nail extending the longitudinal curvature like a ram's horn. Akatsuki disease is a very dirtyskin withgeneralized keratodermatosis. Her dirt was washed off, and her toenails were cut repeatedly with a drill. After treatment with antibiotics and insulin, the akatsuki disease was cured. The causes of onychogryposis were thought to be poor diabetic control, and paronychia with traumatic injury to the fingers. She showed mild abnormality in intelligence tests. The mental retardation and pain due to onychogryposis with paronychia are thought to induce akatsuki disease. Diabetic patients with mental retardation need education in self-cleaning. Help by the families and visiting care by public health nurses should be requested for diabetic patients with mental retardation.
