Effect of octreotide acetate on hypoxemic patient with liver cirrhosis was examined. Our case was a 58 year old man with liver cirrhosis bearing hepatocellular carcinoma. Blood gas analysis showed remarkable hypoxemia of 47.9 torr for PaO2 and 39.1 torr for PaCO2. No abnormalities were found in the chest X-ray, CT or respiratory function, however, shunt ratio measured by 99mTc macro-aggregated scintigraphy was highly 18.4%. The hypoxemic condition of this patient was considered to be hepatopulmonary syndrome. In order to examine long term effect of octreotide acetate on hypoxemia due to hepatopulmonary syndrome, the patient was administered 100μg of octreotide acetate subcutaneously twice a week for 1 month. From 2 to 24 hours after administration, hypoxemic condition improved 10 torr for PaO2, however, no further improvement was seen. Octreotide acetate had no advantage to correct hypoxemic condition of our liver cirrhotic patient in a long duration.
