Abstract

We administered natural interferon-alfa (IFN) in an 18-year-old woman with chronic hepatitis C who had a history of acute lymphoblastic leukemia. She had also been transfused while she received intensive chemotherapy against underlying disease. The titer of serum HCV-RNA was 8.0Meq/ml with branched DNA method and HCV genotype was identified as I+II before IFN therapy. We adopted a long term (6 months) and high dose regimen of IFN therapy. During IFN therapy, 2nd generation antibodies against HCV became negative detection. Serum levels of HCV-RNA were not reduced and abnormal alanine aminotransferase (ALT) levels still remained during IFN therapy and after the cessation of the therapy. The evaluation of viral genome focused on the 5'non-coding region and core region revealed no major mutations. Moreover, frequent alterations in hypervariable region 1 in the amino acid sequences with single-strand conformation polymorphism (SSCP) were not observed during the course of IFN therapy. These findings indicate that IFN therapy in this case did not enhance immune pressure on HCV. We speculated the differences in the immune conditions in this case might be closely associated with both the HCV and inferior response to IFN.
