A percutaneous cardiopulmonary support system (PCPS) was employed in 33 patients (18 males, 15 females, mean age 61.5 years) between January 1991 and 1995. The reasons for employing the bypass were cardiogenic shock (10 patients), cardiopulmonary shock (7), being dead on arrival (4), low output syndrome associated with cardiac surgery (10), and to support percutaneous transluminal coronary angioplasty (PTCA) (2). Activated coagulation time was maintained at 180-200 seconds with systemically administered heparin, nafamostat masilate, or Argatroban. Bypass time varied greatly, ranging from 29 to 8, 640 minutes. Weaning from PCPS was successful in 23 patients (69.7%) and 15 patients (45.4%) are survived. Complications related to the bypass itsself were infrequent, most commonly being associated with arterial-cannula-induced limb ischemia, which could be resolved by careful observation and immediate repositioning. We concluded that the PCPS system is easy to introduce, and can be used rapidly and efficiently in various types of cardiopulmonary failure.
