Thirty-four cases with cervical lymph node metastasis from unknown primary carcinoma were retrospectively investigated regarding the clinical observation and the treatment outcome. With respect to the histopathological types, 27 cases had squamous cell carcinoma (79%) and 7 cases had adenocarcinoma (21%). As for the region of lymph node metastasis with the maximum size, metastasis located in the upper deep cervical region were most frequent (64%). The presence of primary lesions were confrmed in 8 cases (24%) after the treatment of their metastasis lesions. The overall 5-year survival rate as determined by the Kaplan-Meier method was 42%. The 5-year survival rate of 27 cases with their metastatic lesions treated by radical neck dissection was 48%. None of 7 cases with nontreated metastatis that underwent radiation or chemotherapy survived. As for the treatment modalities, the 5-year survival rate of 9 cases with squamous cell carcinoma treated by postoperative chemotherapy was 76%. These results suggested that at least radical neck dissection may be necessary for cervical lymph node metastasis from unknown primary carcinoma. In addition, chemotherapy after surgical treatment was also thought effective for the case with squamous cell carcinoma.
