A 34-year-old man visited our hospital with complaints of low grade fever, abdominal pain and diarrhea in July, 1996. Three days before the onset of symptoms, he ate vinegarred raw fish (mackeral). He has had the symptoms for five days when gastroendoscopic and colonoscopic examinations were performed. Colonoscopic examination showed edematous mucosa of the ileocecal valve and revealed a vivid anisakis larva penetrating mucosa of the ileum 10cm away oral to the ileocecal valve. The mucosa of the terminal ileum was red and edematous. The symptoms disappeared immediately after removal of the anisakis. There has been no reported case of endoscopic treatment of anisakiasis of the terminal ileum, partly because of the difficulty of its diagnosis. Endoscopic observation of the terminal ileum is important in suspected cases.
