A 34-year-old man was admitted because of pain in the right upper quadrant and back. Ultrasonogram of the abdomen and computed tomogram revealed no gallbladder. Agallbladder and a cystic duct were not visualized by three-dimensional imaging with helical CT and ERCP. These findings suggested the possibility of congenital absence of the gallbladder and laparoscopic examination was performed. The gallbladder was absent and only the thin fibrous band was recognized at the gallbladder fossa of the liver. Intraoperative needle cholangiogram revealed neither a gallbladder nor a cystic duct. From these laparoscopic findings we diagnosed this case as congenital absence of the gallbladder.<BR>Only five cases of this disease which were correctly diagnosed by laparoscopy have been reported in Japan. From our experience, laparoscopic examination was useful for making the correct diagnosis of congenital absence of the gallbladder.
