The increase incidence of delayed bleeding after hemorrhoidectomy was evaluated in patients treated with non-steroidal anti-inflammatory drugs (NSAID) for relieving postoperative pain. Three hemorroidal complexes were removed by use of Nd : YAG Laser bipolar dissector from 1, 643 patients under sacral epidural anesthesia.Sixteen out of 708 patients p.o. administered 240 mg zaltoprofen per day and 22 out of 244 patients 225 mg mofezolac per day had bleeding from 4 to 13 days with an avarage interval of 8±2 days postoperatively. Mofezorac increased the incidence of hemorrage significantly (p<0.01). However, 14 out of 691 patients not administred NSAIDs also had bleeding. Ten healthy male volunteers p.o. administered 240 mg zaltoprofen per day for 2 weeks showed a slight impairment of platelet aggregation (p<0.05) only on day 3, without impairment coagulating factors. Eight healthy male volunteers administered 225 mg mofezorac per day for 2 weeks showed significant inhibition of platelet aggregation (p<0.01), prolonged bleeding time (p<0.05), and decrease of fibrinogen (p<0.05). It was concluded that NSAID, which has a potency to inhibit platelet aggregation, influences delayed postoperative hemorrage.
