The results of systematic articulation treatment are reported for a patient who manifested severe apraxia of speech with concomitant mild aphasia. The patient, a right-handed male, had a cerebral infarction when he was 70 years old. Upon initial examination, he could produce a few distorted vowels but no consonants were produced correctly.<BR>Treatment was initiated using the integrated stimulation method. This method uses multimodal stimulation in which the therapist gives clues to the patient on the posture and direction of movements of the articulators and the direction of the air stream, while producing the desired sound. Since correct production of phonemes was observed only inconsistently, the treatment targeted all phonemes and was carried out following the hierarchy of traditional articulation treatment.<BR>During the 8-year course of treatment, the understandability of discourse production in a picture description task improved from 0% to 87.9%. However, in conversational discourse little improvement in intelligibility rating was observed. The integrated stimulation method was thus seen to he efficacious in improving phoneme production in a severely apractic patient but with limited success in carry-over to daily conversation.<BR>While the intelligibility of phonem production improved, the prosodic aspects of speech such as speaking rate deteriorated due to compensatory effort of the patient to articulate phonemes accurately.
