As a local chemotherapy for malignant diseases, cisplatinreleasing bioresorbable microspheres (CDDP-MS) were prepared using a polymer consisting of glycolic acid-L-lactic acid copolymer. An average diameter of CDDP-MS was about 1OO u m. In vitro, all the CDDP was released constantly during the 3 weeks from CDDP-MS. We administered CDDP-MS to the patients with malignant pleural effusions and assayed CDDP in serums and in effusions. After CDDP-MS administration, CDDP was detectable in the serum for two weeks and in the effusion until the time of extubation. CDDP concentrations in serum were always lower than those after the treatment with CDDP solution. Low CDDP concentration in serum contributed to the reduction of the systemic side effects. The total amount of drained CDDP was less than 1% of the administered dose. The cytology of the effusion was changed to negative. CDDP-MS administration required no hydration and the extubation was done on 5.0±1.52 days that was shorter than those treated with CDDP solution. In the control of malignant pleural effusions, CDDP-MS are superior to a CDDP solution and contributes to the improvement of thes patient's quality of life.
