In two patients with ulcerative colitis, who had previously undergone surgery for anovaginal fistula, the normal structure of the anovaginal septum was destroyed. The defect was reconstructed using a gluteal fold flap. After a U-shaped perineal incision, the anterior anal wall was reconstructed by suturing the residual anal sphincter muscles. For the defect of the vaginal posterior wall, a gluteal fold flap was raised. The flap was rotated through the subcutaneous tunnel and then sutured to the defect. The donor defect was then primarily closed, leaving a scar only on the gluteal fold. Postoperatively, there was neither infection nor anovaginal fistula. The gluteal fold flap has the appropriate volume and is useful for reconstruction of the anovaginal septum.
