Abstract

症例は58歳, 男性. 4年前よりC型慢性肝炎を指摘され, 平成11年12月, 黄疸・発熱を契機に肝病変を指摘された. 精査の後, 右葉内多発肝細胞癌と診断され, 肝右葉切除を予定したが, 術後予想肝機能の問題から, 術前PTPEを2回行った後に手術 (肝右葉切除) を施行した. 術後病理学的検索において, 切除肝の後区域ほぼ全域に梗塞性変化が認められたが, S6高分化型肝細胞癌病巣の周囲にのみ非梗塞性肝実質をわずかに残していた. 腫瘍内を還流した血液が腫瘍周囲の非癌肝実質を栄養していたことによると考えられ, 肝細胞癌周囲実質の血流動態, 肝細胞癌の支配動脈・流出静脈系に関し示唆を与える症例と考えられたので報告する.
