Human fatty acid binding protein (HFABP) is specifically plentiful in the cardiac muscle, but it is scarcely involved in the skeletal muscle and other organs. In this study, the usefulness of HFABP as a myocardial impairment index after cardioaortic surgeries was investigated. Subjects were 13 cases of coronary bypass 10 cases of valve replacement with plastic operation, five cases of arch or ascending aorta replacement and one case of throm-bectomy for pulmonary thrombosis.In all cases, HFABP, troponin T, CPKMB, and myoglobin levels were measured before, and immediately, 6 hours, and 24 hours after the operations. CPKMB and troponin T levels revealed a plateau from immediately to 6 hours or to 24 hours after the operation. On the other hand, HFABP showed a highest level immediately after the operation and then rapidly decreased. A significant positive correlation was observed between the highest values of HFABP, troponin T, and CPKMB and the time cutting off the aorta.<br> We think that HFABP is possible to predict myocardial impairment earlier than conventional markers, and sustained increase in HFABP level confirmed by several measurements is good indicator judging the persistence of the impairment.
