抄録

Primary perineal hernia is a very rare type of pelvic floor hernias. A 72-year-old woman was admitted to the hospital in January, 2000. She had been suffering from a tennis ball sized left labial bulging when her abdominal muscle pressure increased at farm works since 1995, and she reduced it each time by herself. Primary anterior perineal hernia was diagnosed, and an operation was performed by abdominal approach. After an incision of the right broad ligament of uterus, a muscular defect in the pelvic floor was found as the hernial orifice. Since primary closure was very difficult, the vaginal wall and urinary bladder wall were sutured at first. Then prolene mesh was sutured between the vaginal wall, urinary bladder wall, uterus and broad ligament, followed by fixation of the broad ligment and uterus to the anterior parietal peritoneum. The postoperative course was uneventful, and the complaint disappeared immediately. There has been no recurrence as of one year after the operation.
