We present a rare case of retroperitoneal abscess caused by fungus after an adrenalectomy. After a laparoscopic adrenalectomy to a 26-year-old woman with left adrenal adenoma, fever and leukocytosis lasted and a retroperitoneal abscess was pointed out by an abdominal CT scan. We performed a CT guided drainage. Subsequently Candida albicans was detected by a bacteriological examination of pus and serum β-D-glucan level was high. Direct injection of antifungal drug into the abscess cavity was started. High fever and increased β-D-glucan level became normalized in week 3 and 4, respectively. No Candida albicans was isolated from a culture and the drain was removed on 117 postoperative day. It is thought that appropriate drainage with direct injection of antifungal drug to abscess cavity may be an optimal therapy for fungal retroperitoneal abscess with minimal side effects.
