Examination was made on 8-year-therapeutic results of thoracoscopic surgeries for pneumothorax in the hospital since October, 1991. A series of 328 cases were treated by thoracoscopic surgery and 238 out of 328 cases, comprising 45 cases of thoracoscopic operation only and 193 cases of 3cm minithoracotomy-combined thoracoscopic operation, were followed for more than 2 years after first operations. Postoperative recurrence was noted in 16 (6.7%) out of 238 cases, seven of 45 cases (15.6%) for the former operation and nine of 193 cases (4.7%) for the latter operation. Of 16 recurrent patients, 11 patients had bilateral spontaneous pneumothorax. Thirteen out of 16 patients underwent re-operation. The causes of recurrence included missing bulla or focus (two cases), ruptured neobulla or emphysematous change area of lung (four cases), injury of the pleura (three cases), catamenial pneumothorax (one case), and unknown origin (three cases). The recurrence after the former operation was ascribable mainly to missed bulla or focus, compared with unknown origin and ruptured multiple neobulla. To reduce the recurrent rate, precise evaluation of the foci before and during operation, appropriate selection of operative procedure according to each patient's conditions, background factors and specific problem, and our surgeon's skills that are enough for its performance are important.
