Abstract

Two cases of undifferentiated carcinoma of the cervical esophagus, treated surgically, are reported. Case 1: An 84-year-old woman was admitted to the hospital because of hoarseness and pharyngeal pain. Cervical esophagectomy and a laryngectomy were performed with a diagnosis of esophageal cancer of Ce, T3, N0, M0, Stage II. Histological examination revealed undifferentiated carcinoma, pT3, pN4(3b), pStage IVa, and curability pB. She had been recurrence free until she died of cardiac failure 29 months after the surgery. Case 2: A 68-year-old man was admitted to the hospital because of a cervical mass and dysphagia. Cervical esophagectomy and a laryngectomy were performed with a diagnosis of esophageal cancer of CeUt, T2, N3 (100-It. int. jugular vein) T4, MO, Stage IV, after neoadjuvant chemo-radio therapy. Subtotal gastrectomy was simultaneously performed for synchronous gastric cancer. The esophageal lesion was undifferentiated carcinoma, pT3, pN0, pStage II, and curability pA. He died of liver metastasis 17 months after commencement of anticancer therapy. Undifferentiated carcinoma rarely occurs in the cervical esophagus. Although undifferentiated carcinoma often metastasizes hematogeneously, surgical resection should be indicated to resolve the local problems such as pain and dysphagia, even if laryngectomy deteriorates quality of life of the patient. Not all of the patients with this condition have a poor outcome.
