Recurrent Jejunal Diverticular Perforations Apparently Induced by Heavy Labor. A Case Report.

Abstract

A 65-year-old man day laborer was admitted to our hospital for sudden abdominal pain. The diagnosis was peritonitis, and emergency surgery was performed, but a small jejunal perforation was discovered, and wedge resection was carried out. Although we did not find any foreign bodies, we diagnosed the perforation as due to something like a fish bone, because there were no other findings in the small intestine. About ten months later, the patient returned with the same complaint, and an emergency operation was performed again. The second operation revealed a similar jejunal perforation near the previous one, and partial resection of jejunum was performed. The laboratory data both times were similar. The LDH and CPK values were very high, and we realized that both perforations had occured after heavy labor by the patient. Fluoroscopy of the small intestine showed only the presence of a few jejunal diverticula, with no underlying diseases, such as Crohn's disease, Behcet's disease, etc. Pathological examination disclosed that the perforations were due to diverticula. We concluded that abdominal muscle pressure as a result of heavy labor may have induced perforations of jejunal diverticula.
