Hepatic portal venous gas is an ominous prognostic sign. We report a case of survival of necrotizing enteritis with hepatic portal venous gas and pneumatosis cystoides intestinalis. A 71-year-old man was admitted with a sudden onset of severe abdominal pain and vomiting. Hepatic portal venous gas was<br>detected by conventional radiography, ultrasonography, and computed tomography. In an emergency laparotomy, the necrotic bowel was resected, and the patient survived.<br> Hepatic portal venous gas suggests the presence of necrotic bowel, so it usually indicates the need for urgent surgical intervention.<br> Since hepatic portal venous gas is difficult to detect by conventional radiography, computed tomography is a more valuable method for this purpose.
