A case of paraesophageal hiatal hernia, which was suspected to recur immediately after an improper surgery, is reported. A 77-year-old woman, who had a Nissen fundoplication for hiatal hernia without functional evaluation at another hospital in March 1995, was admitted to the hospital because of dysphagia, postprandial epigastralgia and weight loss. Barium swallow study revealed that the stomach except the cardia and antrum herniated into the lower anterior mediastinum, which can be called upside-down stomach. Intraesophageal pH and bilirubin monitoring detected no reflux. Laparotomy on March 4 1998 revealed that the hilus of 7cm in diameter was present anterior to the cardia and the content was stomach except the cardia and antrum. The hilus was closed following a resection of the hernial sac. The patient became free from any symptoms and gained weight. Functional evaluation for hiatal hernia is essential when surgical treatment is indicated. Immediate correction is recommended for paraesophageal hiatal hernia to avoid gastric necrosis caused by omenteloaxsic volvulus.
