抄録

A 61-year-old man was admitted to our hospital after a traffic accident. Abdominal CT showed a collection of intraperitoneal fluid, and an emergency operation was carried out. Hemostatic suturing technique was used to control mesenteric hemorrhage. Oral ingestion was started on the 5th postoperative day, but vomiting was reported on the 10th postoperative day, and continuous suction by a nasal gastric tube was started. A computerized tomography scan showed a hematoma. 5cm in size, on the stomach side of the horizontal part of the duodenum. Fluoroscopy also indicated the obstruction was located on the horizontal part of the duodenum. Decompression by a nasogastric tube and intravenous hyperalimentaiton (IVH) were continued for 30 days. Consequently, the obstruction improved, and the hematoma was reduced to 3cm in size. The patient was discharged on the 55th postoperative day. Duodenal obstruction due to hematoma is now adequately treated by conservative methods. We report a case where conservative therapy for about a month was successful.
