A 72-year-old woman was admitted to the hospital because of right lower abdominal pain lasting for a few years. A barium enema study revealed a hemispherical mass projecting into the inferomedial region of the cecum. The appendix was not visualized. Colonscopoy also disclosed a hemispherical mass. Abdominal CT demonstrated a cystic tumor 8.0×6.0cm in size. A laparoscopy assisted surgery was performed with a clinical diagnosis of mucocele of the appendix. The appendix was expanded and contained abundant yellowish white mucin. The mucosa of the appendix was almost deciduated and the remained epithelium was organized in columnar cells indicating no malignant change. The histological diagnosis of the removed material was mucinous cystadenoma of the appendix. Mucocele of the appendix presents difficulty in differentiation between benign and malignant tumors before operation. Except for cases obviously diagnosed as benign, operative procedures which can be expected to result in radical operation should be selected by keeping a probable malignancy in mind. The laparoscopic procedure for mucocele of the appendix is very effective in terms of curability and minimal invasive approach.
