Ten cases of obturator hernia. Usefulness of repair with synthetic mesh.

Abstract

We have experienced ten cases of obturator hernia in the past ten years. The mean age of patients was 77.8 years, ranging from 56 to 87. There were eight women and two men. They were thin, with a mean body weight of 35.6kg. Mean parity of the female patients was 4.3. Howship-Romberg sign was seen in six patients. The most recent seven cases were able to be diagnosed definitely by pelvic CT scan. Incarcerated small intestine in a Richter type of obturator hernia was found in eight cases, and a complete herniation of small intestine was found in one case. Resection of the intestine was performed in three cases. The hernia orifice was repaired with direct peritoneal suture to be covered with the ovary in two cases, suture of obturator in one case, and polypropylene mesh in the most recent seven cases. Pelvic CT scan is very useful in diagnosis of obturator hernia. Repair with synthetic mesh is an easy and effective treatment even if intestinal resection is required.
