Appendical metastasis from gastric cancer is rare and only 15 cases have been reported in the Japanese literature. A 61-year-old man, who had undergone total gastrecotmy, splenectomy and transeverse colectomy for a Borrmann type-3 gastric cancer 4 months ealier, was admitted to the hospital because of a right lateral abdominal pain. The patient was diagnosed as panperitonitis due to perforation of the digestive tract. Emergency operation was performed. At the operation, perforation was observed in the ascending colon. Ascending colostomy and appendectomy was carried out. Histological examination of the submucosa and subserosa of the appendix showed the same signet ring cell carcinoma as that of the gastric cancer. The definite diagnosis of appendical metastasis of gastric cancer was made. When we examine the patient with advanced gastric cancer with acute abdomen, appendical metastasis from gastric cancer should always be considered as a probable diagnosis.
