The great majority of tumors arising in the retroperitoneum are non-epithelial tumor. We present a case of mucinous cystadenocarcinoma, an extremely rare epithelial tumor, of the retroperitoneum, with a review of the literature.<br> A 45-year-old woman was seen at the hospital because of a tumor in the left-lateral abdominal region. The tumor was infant' head-sized and elastic hard with a smooth surface and poor movability. There was no tenderness in the same region. Abdominal imagings revealed a giant cystic lesion with a mural node in the left-lateral abdominal region. It was diagnosed with retroperitoneal cyst, and was excised. The exciced tumor was 17×12×9cm in size, 1350g in weight, oval in shape, and elastic hard. There was a mural node with a papillary protruding inside the tumor. Histologically it was mucinous cystadenocar-cinoma. CEA level in the cystic fluid was extremely high, 48, 000ng/ml. In the treatment of the retroper-itoneal cyst, we should entertain a rare but probable occurrence of mucinous cystadenocarcinoma and perform a complete excision of the cyst, exploration of the cystic fluid, and detailed histological examina-tion of the excised cyst.
