Abstract

A case of asynchronous triple cancer of the stomach, liver and lung was experienced. A 73-year-old man underwent distal gastrectomy with a diagnosis of early gastric cancer in 1991. In 1993, serum CEA increased to 5.3ng/ml, and body CT scan showed S.O.L. in the right lobe of the liver. Ultrasonographyguided liver biopsy specimen revealed a hepatocellular carcinoma. At the same time chest X-p showed a coin lesion in the right middle lobe of the lung. Tomography of the right lung, chest CT, and bronchoscopy suggested primary lung cancer. Right lobectomy of the liver and partial resection of the right lung was performed. Because pathologically these lesions were not metastatic cancer each other, this case was diagnosed as asynchronous triple cancer. In a review of the Japanese literature, 28 resected cases including this one have been reported to have triple cancer preceded by a gastric cancr so far. These multiple cancers are common in men and often involve the gastrointestinal organs. Almost second and third cancers were detected at preoperative examination for the first cancer. Some hereditary factor is generally considered for the multiple cancers, but in this case there was no family history. All three cancers in this case were positive in immunostaining. No chemotherapy was added after operation for the first cancer, and immunological decrease was not confirmed.
