Abstract

A case of ruptured gastric artery aneurysm with polycythemia is reported. A 77-year-old woman was admitted to the hospital because of nausea and vertigo with low blood pressure. After admission, she complained of abdominal pain. Laboratory data indicated leukocytosis, and a plain abdominal X-ray film showed niveaus of the small intestine. Ultrasonography and abdominal computed tomography revealed ascites, so panperitonitis was suspected. Emergency laparotomy showed hemoperitoneum due to a ruptured aneurysm of he gastric artery. The aneurysm was resected. Postoperative course was uneventful. Ruptured gastric artery aneurysm has been reported twenty times in this country, and three of these patients died after surgery. Though this case was of polycythemia and had no findings indicating anemia, we had notice the presence of hemoperitoneum before the general condition of the patient was deteriorated, if abdominocentesis or abdominal angiography was attempted.
