Abstract

A 65-year-old man was admitted to the hospital because of a fever and general fatigue lasting for a month. He had been abroad with sexual intercourse 30 years before. The laboratory tests revealed leucocytosis and mild impairment of the liver function. The TPHA test, HCV antibody and serum amebic antibody were positive. The abdominal CT and ultrasonography demonstrated a solitary abscess 5cm in diameter in the right lobe of the liver. No symptomatic remission could be attained by conservative therapy with Metronidazole. On the 6th day after the admission, percutaneous transhepatic abscess drainage (PTAD) was performed. A few days after the PTAD, the patient got to be afebrile. The microscopic examination of the pus in the liver abscess revealed trophozoite of the Entamoeba histolytica. The colonoscopic examination showed mild redness and erosion in the ascending colon and the trophozoite was seen in the biopsy specimen from the erosion.<br> It seems that the patient had been a cyst carrier of Entamoeba histolytica as sexually transmitted disease, while he was abroad. And this time he may have an amebic liver abscess from the symptomless intestinal amebiasis.
