This paper describes two patients with cancer in the terminal stage whose discomfort was somewhat relieved through the administration of non-opioid analgesic suppositories, continuous extradural blocks, administration of long-acting morphine hydrochloride tablets, continuous IV administration of morphine, local administration of narcotics into the “keiketsu”, and acupuncture.
Case 1: A 77-year-old man with inoperable cystic adenocarcinoma of the pancreas with hepatic metastasis suffering from abdominal pain and lumbago. Additional discomfort described as a heavy sensation of the shoulders and neck was relieved by local narcotic infiltration of the “keiketsu” and/or a continuous extradural block using a portable continuous infusion pump. With treatment, the patient was able to remain at home.
Case 2: A 58-year-old man with metastasis of the left kidney and lymph nodes following surgery for gastric cancer. Discomfort included lumbago, headaches, and abdominal pain. As the effects of continuous extradural blocks diminished, acupuncture was employed to relieve the headaches and abdominal pain. The continuous IV administration of morphine and regular intake of long-acting morphine tablets improved his pain levels from VAS8 on admission to VAS4. This lessening of discomfort resulted in, at least temporarily, a renewed interest in life on the part of the patient.
