Clinicopathological features and long term results of 23 resected cases of hepatocellular carcinoma (HCC) with chronic hepatitis (CH group) were studied in comparison with those of 81 cases of HCC with liver cirrhosis (LC group).<br> There were no significant differences in the histological findings except for the size of HCC between the two groups (CH group; 64±38mm, LC group; 44±26mm). The preoperative tests of the liver function (ICGR15 and prothrombin time) were significantly better in CH group than in LC group. Major hepatectomies, whose extent of hepatic resection exceed 2 hepatic segments, were more oftenly employed in CH group (61%) than in LC group (33%). CH group showed better 5-year cumulative survival rate (81.8%) and recurrence rate (25.5%), than those in LC group (42.1%, 82.4%), respectively. These results were considered to be attributed to followings; (1) Major resection of the liver could offer better curability of HCC. (2) The HCC arised from multicentric carcinogenesis seemed not to be feasible in CH group, whereas in LC group this multicentricity might accout for some of the recurrence.<br> It is concluded that major hepatic resection should be employed in patients with HCC in CH.
