Abstract

A 19-year-old male, who had been under treatment at another hospital with a diagnosis of Crohn's disease, was admitted to the hospital because of a right lower quadrant pain and right thigh pain. A fist sized mass was palpable with tenderness and defense musculaire in the right lower quadrant on admission. There was an iliopsoas sign in the right thigh. Abdominal CT revealed a heterogenous mass about 6 cm in diameter in the ileocecal lesion and a low density lesion with free air in the right iliopsoas muscle. Ileocecal excision and drainage of the right psoas abscess were performed with a diagnosis of psoas abscess complicating by Crohn's disease. It has been reported that psoas abscess concurred in 2.7-10% of patients with Crohn's disease in the Western countries, but its frequency is very low in Japan. A case of Crohn's disease complicated by psoas abscess is so rare that only 2 cases including this case can be seen in the Japanese literature. This rare case is presented here with a review of the literature.
