We experienced a case of hemangiopericytoma of the uterus with tumor thrombus of the left internal iliac vein.<br> In February, 1992, a 43-year-old woman was admitted to the hospital because of a lower abdominal tumor. Hysterectomy was performed with a preoperative diagnosis of hysteromyoma and histopathologically the patient was diagnosed as having a hemangiopericytoma of the uterine. Postoperative chemotherapy with CDDP, CPA, and 5-FU was added 12 times, however, the tumor recurred and enlaged. CT visualized a 6×3cm intrapelvic tumor with invasion into the bladder, dilation of the left ureter, and hydronephrosis. On November 16, 1993 a lower abdominal midline incision was performed, and a hard and adult fist-sized tumor dark-red in color was detected on the floor of the pelvis which invaded widely covering the left-sided wall of the pelvis. Extripation of the tumor with resection of the left ureter, excision of the rectum, and associated excision of the bilateral ovaries and bladder was carried out. Tumor thrombus of the left common iliac vein was found and it was removed and revascularized. There has been no sign of recurrence, as of 2 years after the operation.<br> The patient is followed on an ambulant basis.<br> Hemangiopericytoma of the uterine has been reported in only four cases including our case in Japan. Among of them tumor thrombus was able to be proved only in this case, but early and aggressive operation would be necessary.
