Abstract

A 33-year-old woman was diagnosed as to have a presacral dermoid cyst at a maternity hospital where she gave birth to the second infant, and was referred to ACC for surgical treatment. On digital examination, subcutaneous masses were palpated on the right posterior wall of the rectum. Pelvic CT and MRI revealed three uncontinuous cystic lesions that extended from presacral space to the perianal region. With a preoperative diagnosis of multiple dermoid cysts, resection of the lesions carried out through a combined abdominal and transsacral approach. Further smaller nodules were detected in perianal subcutaneous region in addition to the masses pointed out preoperatively, and were also exicised. The pathological diagnoses of all specimens were dermoid cysts.<br> Congenital cysts that arise in the presacral space, including the dermoid cyst featured in the current report, are collectively known as development cyst. Presacral development cyst is rarely found in adults, and only 30 cases including ours have been reported in the Japanese literature. Of these, only 2 cases were found to be multiple development cysts. By reviewing these reports, we have come to the conclusion that it is almost impossible to decide whether the lesion is benign or malignant prior to the operation.
