PREOPERATIVE MORPHOMETRIC APPROACH FOR THE DIAGNOSIS OF FOLLICULAR CARCINOMA OF THE THYROID

Abstract

The present study was carried out to evaluate the preoperative quantitative estimations of various nuclear features which were obtained by means of fine-needle aspiration biopsies, for the diagnosis of follicular carcinoma of the thyroid. Subjects were 48 patients with follicular tumor of the thyroid operated on at the hospital between November 1987 and December 1994, whose histological explorations were fully conducted. Morphometric parameters including nuclear area, nuclear perimeter, circular rate, the largest to the smallest diameter ratio (LS) and coefficient of variation in nuclear area (NACV) were measured and calculated in aspirated cytological smears obtained from the patients with follicular tumor of the thyroid (18 follicular carcinomas, 13 adenomas and 17 adenomatous goiters). All parameters were estimated in at least 100 nuclei per case, and the results were analysed statistically. The circular rate of nuclei from adenomatous goiters was significantly greater than follicular carcinomas (p<0.001) and adenomas (p<0.05). LS of follicular carcinomas was significantly greater than adenomatous goiters (p<0.05). NACV showed significant differences between any of the groups (p<0.001).<br> In conclusion, preoperative quantitative estimations of cytological nuclear features are of great help concerning differential diagnosis of follicular tumors of the thyroid.
