抄録

We experienced an adult patient with Hirschsprung's disease that had been treated until recently when the patient was emergently transferred to the center because of dyspnea due to a huge mass of accumulated stool in the sigmoid colon which weighted about 30kg.<br> The patient was a 28-year-old woman, who had been suffering from constipation from childhood and had been recommended to undergo surgical therapy, but she had deneyed. On October 24, 1995, she was seen at the hospital because of dyspnea due to significant abdominal distention. Abdominal CT on admission revealed dilatation of the sigmoid colon which occupied the entire abdomen and a huge mass was confirmed in the colon. Emergency laparotomy disclosed that the sigmoid colon was filled with a clayey huge mass of stool about 30kg in weight. Sigmoidectomy and Hartmann operation were carried out. With a biopsy of the rectum after the operation, the definite diagnosis of Hirschsprung's disease was made.<br> No such a case that accumulated stool mass due to Hirschsprung's disease causes shock state has been reported so far, as far as we could review. This case is thought to be extremely rare.
