Herniography was performed to improve diagnosis prior to surgery in a 9-year-old male patient, who presented a left groin bulging mass not typical for indirect hernia. Under local anesthesia, peritoneal needle-puncture was performed with ultrasonographic guidance. Herniograms were obtained after an injection of 50-ml non-ionic contrast medium under fluoroscopy. The total examination time was approximately 20 minutes. No complication was encountered. Scrotal hernia in the left side and patent processus vaginalis in the right side were clearly demonstrated. Both sides were then properly repaired simultane-ously in consideration of those herniographic findings.<br> Herniography can be performed simply and safely even on children, who can tolerate abdominal puncture under local anesthesia. This examination provides useful information for selection of proper surgical method preoperatively, especially in cases with diagnostic difficulties. It is supposed to be indicated selectively for older children whose clinical manifestations are not typical for congenital inguinal hernias.
