We have experienced a case of extremely rare carcinosarcoma of the breast. A 76-year-old woman was admitted to the hospital because of a left giant breast tumor. The tumor was 10×10cm in size and occupied the almost entire quadrants of the left breast with skin ulceration. And metastases to the left axillary lymph nodes and right breast were recognized. Bilateral modified radical mastectomies and full-thickness graft to left chest wall were performed. Histological examination revealed of ductal carcinoma and stromal sarcoma. Immunohistochemical staining was performed. The carcinoma component stained positive for keratin and epithelial membrane antigen (EMA). Vimentin was negative. The sarcoma component stained positive for Vimentin. Keratin and EMA were negative. Transitional foci between two components were not observed. And there were no metaplasia of cartilagonous and osseus type. We diagnosed this tumor was carcinosarcoma in a narrow sense. Metastases in the left axillary lymph nodes and right breast were consisted of carcinoma only. This case of breast carcinosarcoma is very rare as well as very interesting.
