抄録

We present a very rare case of acute emphysematous cholecystitis associated with pneumobilia. A 74-year-old man with previous histories of diabetes melitus, hypertension and transient ischemic attack was admitted to the hospital because of right hypochondralgia. Plain abdominal X-ray film and CT scan showed air in the gallbladder and bile duct. The patient was diagnosed as acute emphysematous cholecystitis and on the dya of admission an emergency laparotomy was carried out. The gallbladder markedly swelled and was dark brown in color, indicating gangrenous cholecystitis. Intraoperative cholangiography revealed no abnormal findings, so a cholecystectomy and abdominal drainage were performed. Seventeen mixed stones, 1 to 3mm in diameter, were found in the gallbladder. With cultivation of th bile juice collected during the operation, Fusobacterium species were detected. Pathologically, hemorrhage, necrosis, cell nests of neutrophiles and air bubbles in the gallbladder wall were observed. The postoperative course was uneventful. He was discharged from the hospital on the 23rd postoperative day. This is the tenth case of acute emphysematous cholecystitis associated with pneumobilia in Japan.
