We report a case of late onset of benign bile duct stricture secondary to a blunt abdominal trauma with a review of 53 cases seen in the Japanese literature. A 50-year-old woman was seen at the hospital because of jaundice. She was involved in an automobile accident, suffering a steering wheel injury with liver lesion, and was treated conservatively with a remission six months before. Blood chemistry tests showed a total builirubin of 12.1mg/dl, CEA of 6.2ng/ml and increased to CA19-9 of 176.6U/ml. Abdominal ultrasonography and computed tomography showed dilatation of the common bile duct and an irregular mass in the pancreas head. Endoscopic retrograde cholangiopancreatography showed the Santorini's duct deviated to the lower side and severe stricture of the lower, bile duct. At laparotomy we found a 2.5cm hard mass involving the lower bile duct, and with a suspicion of bile duct cancer a pancreatoduodenectomy was performed. Microscopy revealed no malignant lesion but reactive infiltration of giant cells and lymphocytes, and fibrosis probably due to bile leakage, permitting the diagnosis of benign the bile duct stricture secondary to blunt abdominal trauma.
