Abstract
A case of Castleman's disease of the retroperitoneum which presented difficulty in preoperative diagnosis is reported.<br> A 34-year-old woman was seen at the hospital for close examination because abdominal ultrasonography revealed a tumor at the hilum of the spleen. Ultrasonography and magnetic resonance imaging (MRI) suggested a possibility of accessory spleen. However, on dynamic CT and angiography the tumor was visualized as homogenous and strong accumulation of a contrast material in the initial phase, and then the accumulation rapidly disappared to that of the spleen. Thus the possibility of accessory spleen could be ruled and non-functioning islet tumor arising in the pancreatic tail was suspected. An operation was carried out, and histopathologically it was diagnosed as hyaline-vascular type of Castleman's disease. In this case we had difficulty in making preoperative quality diagnosis. If we entertain the possible presence of the disease, however, dynamic CT and angiography should contribute to make the definitive preoperative diagnosis.
