MRI diagnosis of the TMJ in orthodontic patients

Abstract

Abnormalities of dental occlusion have been considered to be the main etiology of temporomandibular joint (TMJ) disorders. As such, some orthodontic patients have TMJ disorders. These patients require frequent evaluation of imaging, and they should be examined during orthodontic treatment. Magnetic resonance (MR) imaging is the primary imaging modality for patients with symptoms of TMJ disorders. However, various orthodontic appliances may limit the usefulness of MR imaging, because they cause imaging degeneration. The purpose of this study was to examine the metal artifacts on MR images of a variety of orthodontic appliances (11 types), using a phantom, and to evaluate the MR images of TMJ patients with orthodontic appliances (5 types), in clinical experiments.<BR>The following results were obtained:<BR>1. Most orthodontic appliances, such as a bracket, a hook, a tube attachment, and wire, wire produced severe metal artifacts, except for a ceramic bracket, wire of NiTi on phantom study. These orthodontic appliances were produced metal artifacts ranging from 0.4cm to 11.9cm on phantom study.<BR>2. In the clinical experiments, it was possible to examine the TMJ on MR imaging in Type A.<BR>Although most patients with orthodontic appliances can examine the TMJ using MR imaging, the authors recommend that patients with orthodontic appliances use a surface coil and remove orthodontic appliances of molars and wire, for accurate MR examination.
