抄録

Pneumatosis cystoides intestinalis rarely presents intraabdominal free air that poses a probrem in differential diagnosis to indicate emergency operation. This paper presents a patient with pneumatosis cystoides intestinalis of small intestine performed a laparotomy with a suspicion of perforated gastric ulcer, who had histories of gastric ulcer and schizophrenia.<br> A 44-year-old man was admitted to the hospital because of abdominal pain. abdominal X-ray film revealed intraabdominal free air under the bilateral diaphragma, and an emergency operation was performed. During operation, yellowish white ascites was confirmed, but no perforated site was demonstrated. Since there were numerous emphysematous changes in the submucosa and mesentery covering from the jejunam about 1m to the Treitz ligament to terminal ileum, portions manifesting strong emphysematous changes were resected. Histopathologicaly, prominent emphysematous cysts involving from muscular layer of mucosa were confirmed, and pneumatosis cystoides intestinalis was definately diagnosed. The patient's postoperative course was uneventful.
