抄録

A 64-year-old woman, who had been followed for 20 years after having infected with hepatitis type B virus at another hospital, was found to have hepatocellular carcinoma (HCC) at the dome of the liver in S8. Despite percutaneous microwave coagulating therapy two times and then TAE, MRI revealed recurrence of HCC. The patient was referred to the hospital. After admission to the hospital, we performed MCT with a thoracoscopic transdiaphragmatic approach. Though it was difficult to approach the HCC at the dome of the liver percutaneously, thoracoscopic transdiaphragmatic approach provided a good surgical view and an easy access to the tumor. We could get enough area of coagulations. No local recurrence of HCC has occurred.
