We present a case of an azygous vein injury (AVI) in blunt chest trauma. A 47-year-old woman was involved in a traffic accident, she was injured by the side-impact collision and was brought to the emergency room without any sign of massive hemorrhage. Thirty minutes after arrival, since computed tomography showed right hemothorax, right tube thoracostomy was performed. After 700ml of blood was drained through the chest tube, her systolic blood pressure fell to 70mmHg. Because the bleeding gradually increased via drainage tube, we decided to perform thoracotomy. In the operation we found that the azygous vein was injured in the azygous arch and in the portion between the confluences of 5th and 6th intercostal veins. Thoracic wall injury was not found. Hemostasis was achieved by the suture ligation of the injured sites. The patient was discharged from hospital without any complication caused by hemorrhagic shock 49 days after operation. Only 13 cases of AVI have been reported before our case. Increased hemorrhage caused by chest drainage and delayed hemothorax were reported as characteristic feature of AVI. It might be noted for initial evaluation of chest trauma patients to prepare the potential risk of massive hemothorax caused by AVI.
