咀嚼筋の機能の定量的評価の可能性を明らかにする目的で, 20歳代の女性正常者20名と女性側頭下顎障害 (TMD) 患者20名に最大クレンチングの80%の強さのクレンチング (80%MVC) を20秒間行わせ, クレンチング開始後の5秒間ごとの4区間 (第1~第4区間) における咬筋のEMGパワースペクトルの累積100%値, 累積80%周波数値, MPF値について, 両群間で比較した結果, 第1区間を100%とした場合の第2~第4区間の累積100%値は, 両群ともに100%に近似し, 80%MVCを20秒間維持できることが認められた。また, 累積80%周波数値とMPF値は, 正常群では, 第1区間が最も大きく, 第2区間が著明に減少し, その後第3, 第4区間の順に徐々に減少する傾向を示したが, TMD群では, 第1区間が最も大きく, その後第2, 第3, 第4区間の順にわずかに減少する傾向を示し, 時間的推移が正常群とは明らかに異なり, 両群間に有意差が認められた。さらに, 第1区間を100%とした場合の第2, 第3, 第4区間の累積80%周波数値とMPF値は, いずれの区間でもTMD群のほうが正常群よりも大きく, それぞれ両群間に有意差が認められた。これらのことから, 20秒間の80%MVC時における咬筋のEMGパワースペクトルの累積80%周波数値とMPF値は, 咀嚼筋の機能の定量的評価の有効な指標になることが示唆された。

The purpose of this experiment was to investigate the possibility of quantitatively evaluating masticatory function. Twenty healthy subjects and 20 TMD patients in their twenties were selected. The masseter muscular activity during 80% clenching (80% MVC) of the maximum clenching for 20 seconds was recorded with EMG. The 20-second clenching was divided into four 5-second periods, and for each period the cumulative 100% value, the mean power frequency (MPF) value and the cumulative 80% frequency (80% F) value of EMG power spectrum were calculated and were compared between healthy subjects and TMD patients. When the first period was set to 100%, the cumulative 100% values for the second, third and fourth periods were all close to 100% also for both groups which indicated that they both can maintain 80% MVC for 20 seconds. In the healthy subject group, the MPF and 80% F values for the first period was the largest and decreased dramatically for the second period and decreased slightly for the third and fourth periods. On the other hand, for the TMD patient group the MPF and 80% F values were the largest for the first period and thereafter decreased gradually from the second period to third and fourth periods. They clearly showed a different pattern from the healthy group and a significant statistical difference was found between the two groups. When the first period was set to 100%, the MPF and 80% F values for the second, third and fourth periods were much larger for the TMD patient group than for the healthy subject group and a significant statistical difference was found between the two groups. It was concluded that the MPF and cumulative 80% frequency values of the EMG power spectrum at 80% MVC for 20 seconds could be used as a quantitative indicator for evaluating masticatory function.
