We investigated patient with colorectal cancer living in the Awaji-shima Island where the aging of the proportion is remarkable and senior citizens [65 and older] account for 22.4%. The number of operations for colorectal cancer in a recent 11-year period was 766 and an averge age of the patients was 67.3 years old. Consistent with the aging patterns of the residents of this area, there were many elderly patients. The number of patients, over 70 years old was 334 (43.6%); the number of patients, over 80 was 121 (15.8%). Among the patients, over 70, the number of the patients requiring emergency operations was 149 (19.5%). Of these, 113 patients (14.9%) suffered from obstruction of the large intestine and 33 (4.3%) from perforation of the large intestine. The proportion of emergency operations increased in accordance with patient age; it was 15.5% in patients under 70, 20.2% in patients in their seventies, a nd 33.9% in patients over 80. As mass screening for colorectal cancer has been permeated, the proportion of operations for colorectal cancers in a relatively early stage has recently increased. The number of early stage colorectal cancers treated by endoscopic mucosal resection came to 295. However, the Stage-IV cases remained at about 10% overall; this increased to 24.8% in patients over 80. The mortality rate of patients during the operation was 0.7% in patients under 70, 1.8% in those in their seventies and 4.1% in those over 80. The five-year survival rates after operation ware 67.4% in patients under 70, 64.8% in those in their seventies and 44.8% in those over 80. Even the survival rate of the patietns over 80 years old was almost 70% of those of the younger age groups, five-year survival rate remained 50% and is equivalent with the younger age groups.
