We present a case of portal vein aneurysm with metachronous double cancer of the stomach and sigmoid colon. A 68-year-old man was referred to the hospital for evaluation of a gastric cancer. Abdominal CT and ultrasonography (US) demonstrated a portal vein aneurysm at the junction of the splenic and superior mesenteric veins, and it was confirmed during operation. Two years later he was readmitted to the hospital because of anal bleeding. Lower gastrointestinal series showed a sigmoid colon cancer. CT, US, and angiography disclosed no change in size of the portal vein aneurysm. Color doppler US revealed laminar flow. So we decided to observe the course of the aneurysm continuously without any teatment.<br> In a review of the literature, no cases causing rupture of the aneurysm or fatal complication have been reported among those of portal vein aneurysm without portal hypertension. For such cases including this case, the observation of the course would be permitted.
