Enteritis necroticans caused by Clostridium perfringens type C, which is known as Pig-bel or Darmbrand, is a common life-threatening disease in developing countries such as Papua New Guinea. However, it is rarely encountered in advanced countries such as Japan. We experienced a case of this disease in a 72-year-old male who underwent emergent operation with a diagnosis of idiopathic perforation of the ileum. Enteritis necroticans was correctly diagnosed by bacteriological and histological identification of Clostridium perfringens by culture of abdominal fluids and resected materials from the small intestine.
